Hospital Benefit Claims

Documents Required

  1. Duly completed Hospital Benefit Claim Form
  2. Duly completed Attending Physician's Statement Medical report (Part B)
  3. Hospital bill/discharge note showing admission and discharge date
  4. Admission and Discharge Card
  5. Laboratory and Radiology Reports

  1. Duly completed Accident Claim Form
  2. Duly completed Attending Physician's Statement Medical report (Part B)
  3. Appendix A duly completed by Employer
  4. Original Medical Certificate(s) (if Life Assured is self-employed)
  5. Certified True Copy of Medical Certificate(s) (if Life Assured is under employment)
  6. Original official receipt(s) (for reimbursement benefit only)
  7. Original itemized bill(s) (for reimbursement benefit only)
  8. Police report

  1. Duly completed Medical Benefit And Hospital & Surgical Claim Form
  2. Duly completed Attending Physician's Statement Medical report (Part B)
  3. Original official receipt(s) (including deposit and refund receipt, if any)
  4. Original itemised bill(s)
  5. Laboratory and Radiology Reports
  6. For Overseas claims:
    a. Certified True Copy of passport indicating Biodata, Dates of Departure from Malaysia and Arrival overseas
    b. Original detailed admission bill and receipt (translation of foreign language to English)
  7. Original itemized bill(s) (for reimbursement benefit only)
  8. Police report

  2. Duly completed Attending Physician's Statement on Critical Illness Claim
  3. Certified true copy of Diagnostic report, e.g. biopsy, CT scan, x-ray, if available
  4. Original Policy Contract
  5. Proof of age, if not admitted (certified IC, passport or birth certificate)

  2. Duly completed Attending Physician's Statement on TPD claim

  5. Proof of age, if not admitted (certified IC, passport or birth certificate)
  6. Medical certificate, if available
  7. Police report, if any
  8. Newspaper cutting, if available

  6. Certified Death Certificate
  7. Police report for accidental death
  8. Certified post mortem report for accidental death
  9. Proof of relationship
