- Duly completed Attending Physician's Statement on Death Claim (where applicable)
- Certified true copy of Diagnostic report, e.g. biopsy, CT scan, x-ray, if available
- Original Policy Contract
- Proof of age, if not admitted (certified IC, passport or birth certificate)
- Certified Death Certificate
- Police report for accidental death
- Certified post mortem report for accidental death
- Proof of relationship
- Newspaper cutting, if available
